What is Mounjaro (Tirzepatide) for Weight Loss?

Mounjaro® (tirzepatide) is a once-weekly injectable medication developed by Eli Lilly. Since November 2023, the MHRA (Medicines and Healthcare products Regulatory Agency) has authorised Mounjaro for use in adults with obesity (BMI ≥30) or overweight (BMI ≥27) with at least one weight-related comorbidity, such as hypertension, dyslipidaemia, or obstructive sleep apnoea. Mounjaro represents a significant advancement in the medical treatment of obesity, offering a new therapeutic option for individuals struggling to achieve and maintain a healthier weight.

Benefits of Weight Loss

Losing weight offers numerous health benefits that can improve both your quality of life and long-term well-being. Achieving and maintaining a healthy weight helps reduce the risk of chronic conditions such as type 2 diabetes, heart disease, high blood pressure, and stroke. Weight loss can also improve joint health, reducing pain and stiffness associated with arthritis.

Additionally, weight loss can often lead to better sleep quality, increased energy levels, and enhanced mental health by lowering stress, anxiety, and reducing depression-like symptoms. For many, weight loss boosts confidence and supports a more active lifestyle, enabling greater participation in everyday activities and hobbies. Overall, effective weight management is a key step toward living a healthier, longer life.

How Mounjaro Works for Weight Loss

Mounjaro is a dual GIP and GLP-1 receptor agonist. It works by mimicking the actions of two key gut hormones:

  1. GIP (glucose-dependent insulinotropic polypeptide): Supports fat metabolism and insulin sensitivity.
  2. GLP-1 (glucagon-like peptide-1): Suppresses appetite, enhances satiety, and slows gastric emptying.

This dual hormone action helps reduce hunger, regulate calorie intake, and support sustained weight loss over time. Clinical trials have shown that tirzepatide can lead to substantial and clinically meaningful reductions in body weight, especially when combined with a reduced-calorie diet and increased physical activity.

Is Mounjaro Right for You?

Mounjaro is available by prescription and should be used under the guidance of a qualified healthcare professional. It is typically considered when lifestyle interventions alone are insufficient to achieve weight loss goals. Treatment is most effective when combined with long-term lifestyle changes, including dietary improvements and regular physical activity.

How is Mounjaro Taken?

Mounjaro (tirzepatide) is administered as a once-weekly subcutaneous injection, typically into the abdomen, thigh, or upper arm. The injection is delivered using a pre-filled multi-dose KwikPen containing 4 weekly doses, designed for easy at-home use. Patients can administer the injection themselves after receiving appropriate guidance from a healthcare provider. Mounjaro can be taken at any time of day, with or without food, and should be injected on the same day each week. This once-weekly injection makes Mounjaro a practical option for people managing busy schedules or those who are new to injectable weight loss treatments.

Dosage and Titration Schedule

Mounjaro is started at a low initial dose to help minimise side effects, particularly gastrointestinal symptoms such as nausea. The typical starting dose is 2.5 mg once weekly for four weeks. After this period, the dose is gradually increased in 2.5 mg increments every four weeks, based on individual response and tolerability. The maximum recommended dose for weight loss is 15 mg once weekly. This titration schedule allows the body to adjust to the medication while maximising both safety and effectiveness. Your healthcare provider will closely monitor your progress and adjust the dosage accordingly.

What Does the Research Say About Mounjaro?

Mounjaro has been extensively studied in both diabetes and weight loss populations. In the SURMOUNT clinical trials, tirzepatide demonstrated superior weight loss results compared to other medications, including GLP-1 receptor agonists like semaglutide (Wegovy). Participants without diabetes lost an average of 15% (5 mg) to 21% (15 mg) of their body weight over 72 weeks. These results have positioned Mounjaro as a leading treatment for obesity, now licensed in the UK for this indication. Ongoing research continues to evaluate its long-term safety, metabolic benefits, and potential role in preventing obesity-related conditions such as type 2 diabetes, heart disease, and sleep apnoea.

Mounjaro and Contraception

The effectiveness of birth control pills may be reduced when starting Mounjaro (tirzepatide) or after any increase in your dose. To ensure full contraceptive protection, it is essential to use an additional barrier method of contraception, such as condoms, or switch to a non-oral contraceptive method like an intrauterine device (coil) during this time. You should continue using these additional or alternative contraception methods for at least four weeks after starting Mounjaro, and for four weeks following any dose escalation. Always discuss your contraception options with your healthcare provider before beginning treatment with Mounjaro.

Mounjaro (Tirzepatide) and Pregnancy or Breastfeeding

Mounjaro must not be used or initiated during pregnancy, as its effects on an unborn baby are not known. Use effective contraception while you are using Mounjaro. If you are pregnant or suspect you might be pregnant, do not start or continue Mounjaro treatment, and consult your healthcare provider immediately to discuss safer alternatives. If you are planning a pregnancy, speak to us or your GP for advice before you start Mounjaro, or before you stop it if you are already using it. Similarly, Mounjaro should not be used while breastfeeding, as its effects on a nursing baby are unknown. If you are breastfeeding, talk to your healthcare provider before using Mounjaro to explore safer options for weight management.

How Are Wegovy and Mounjaro Different?

Mounjaro (tirzepatide) and Wegovy (semaglutide) are both injectable treatments used to support weight management, but they work in slightly different ways and may suit different patients. Wegovy is a GLP-1 receptor agonist, while Mounjaro targets both GLP-1 and GIP receptors, which may offer added benefits for some individuals. Both medications are taken once weekly and can be highly effective when combined with lifestyle changes. Choosing between them depends on factors such as treatment goals, side effect profile, availability, and individual response, which is why professional guidance is essential.

Side Effects of Mounjaro

While Mounjaro (tirzepatide) is an effective treatment for weight management, it is important to be aware of potential side effects, both common and serious. Common side effects may include nausea, diarrhoea, decreased appetite, vomiting, constipation, indigestion, and stomach pain. These symptoms are typically mild to moderate in intensity and often improve over time. To minimise discomfort, healthcare professionals recommend gradual dose escalation, also known as titration, which allows the body to adjust to the medication in a stepwise manner.

Serious side effects: what to look out for and what to do

Although we don't expect most people to get these side effects, as they are less common, we still want you to know their signs and symptoms so you can get help promptly if ever needed.

1. Serious allergic reaction (anaphylaxis)

Signs and symptoms: swelling of the face, lips, tongue or throat, difficulty breathing or swallowing, wheezing, a severe rash or itching, feeling faint or dizzy, or a very fast heartbeat.

What to do: stop using Mounjaro and do not take another dose.

Where to get help: call 999 immediately.

2. Inflammation of the pancreas (pancreatitis)

Signs and symptoms: severe, persistent pain in the upper abdomen that may spread to your back, with or without nausea and vomiting.

What to do: stop using Mounjaro. If pancreatitis is confirmed, you must not restart it.

Where to get help: call 999 or go to A&E. Let us know once you have been seen.

3. Gallbladder problems

Signs and symptoms: pain in the upper right side of the abdomen, fever, yellowing of the skin or eyes (jaundice), or pale, clay-coloured stools.

What to do: do not take your next dose until you have been assessed.

Where to get help: contact your GP urgently or call NHS 111. If the pain is severe or you have yellowing of the skin or eyes, go to A&E. Let us know the outcome.

4. Dehydration

Signs and symptoms: ongoing vomiting or diarrhoea, feeling very thirsty, a dry mouth, passing little or dark urine, dizziness, or tiredness.

What to do: drink plenty of fluids, especially when you start treatment or increase your dose, and do not take your next dose until the symptoms have settled.

Where to get help: contact our clinical team or call NHS 111. If you cannot keep fluids down or are passing very little urine, see your GP the same day or go to A&E.

5. Severe or persistent stomach problems

Signs and symptoms: nausea, vomiting, stomach pain, diarrhoea or constipation that is severe or does not improve.

What to do: do not increase your dose, and keep drinking fluids.

Where to get help: contact our clinical team. We may need to lower your dose. If symptoms are severe, call NHS 111.

6. Low blood sugar (hypoglycaemia) (more likely if you take other diabetes medicines)

Signs and symptoms: shakiness, sweating, feeling hungry, dizziness, confusion, blurred vision, a fast heartbeat, or irritability.

What to do: eat or drink something sugary straight away, such as glucose tablets, a small glass of fruit juice or a non-diet fizzy drink, then have a snack once you feel better.

Where to get help: tell your GP or diabetes team, as your other diabetes medicines may need adjusting. Call 999 if someone becomes drowsy, cannot swallow or loses consciousness.

7. Changes in vision (mainly if you have diabetic eye disease)

Signs and symptoms: blurred or changing vision, new floaters, or dark patches.

What to do: keep your regular diabetic eye screening appointments.

Where to get help: tell your GP or optometrist promptly. For sudden loss of vision, go to A&E.

8. Changes in mood

Signs and symptoms: new or worsening low mood, anxiety, irritability, or loss of interest in things you usually enjoy.

What to do: do not ignore these changes, even if they seem mild.

Where to get help: contact our clinical team or your GP. If you have thoughts of harming yourself, call 999 or go to A&E. You can also call Samaritans free on 116 123, day or night.

9. Thyroid (precaution)

Signs and symptoms: a lump or swelling in the neck, hoarseness, difficulty swallowing, or shortness of breath.

What to do: tell your prescriber if you or a close family member have ever had medullary thyroid cancer (MTC) or MEN 2. Thyroid tumours were seen in animal studies, but it is not known whether this applies to humans.

Where to get help: if you notice any of these symptoms, contact your GP promptly and let us know.

10. Surgery or sedation

Why it matters: Mounjaro slows how quickly your stomach empties, which can increase the risk of food entering your lungs during a general anaesthetic or deep sedation.

What to do: tell every doctor, dentist or nurse treating you that you use Mounjaro before any planned procedure.

Where to get help: your surgical or anaesthetic team will advise whether to pause treatment beforehand.

Contact our clinical team: 0121 752 7584 or help@iqdoctor.co.uk. In an emergency, always call 999.

Patient Counselling

  • Tirzepatide weight loss injections - such as Mounjaro - must be taken with a recommended healthy, balanced diet and a regular physical exercise routine.
  • Watch our online video to learn proper weight loss injection techniques.
  • Mounjaro (tirzepatide) should be injected on the same day each week, at any time of day, and can be injected before or after food.
  • The dosage for this weight loss medication needs to be tapered up to avoid any gastrointestinal side effects. Start at 2.5 mg once weekly for 4 weeks, then increase in 2.5 mg steps at least every 4 weeks. Maintenance doses are 5 mg, 10 mg or 15 mg; maximum 15 mg once weekly.
  • Inject into the front of the thigh, the back of the upper arm, or the abdomen at least 5 cm away from your belly button. Rotate injection sites each week.
  • If a dose is missed, and it has been 4 days (96 hours) or less since your missed dose, take it as soon as you remember. If it has been more than 4 days, skip the missed dose and take your next dose on your usual day. Either way, then carry on with your normal once-weekly schedule.
  • If you miss more than one dose in a row, do not restart at your usual dose without speaking to us first. Contact our clinical team, as you may need to restart at a lower dose to reduce the risk of side effects.
  • You can change the day you take your weekly injection if you need to, as long as there are at least 3 days (72 hours) between two doses. Once you have chosen your new day, carry on injecting once a week on that day.

Special Warnings and Precautions for Use of Mounjaro

Serious side effects, contraception and surgery are covered in the sections above. Please also note the following.

History of pancreatitis or severe stomach conditions

Tell us during your consultation if you have ever had pancreatitis, or if you have a severe gastrointestinal condition such as gastroparesis, as tirzepatide has not been studied in people with severe gastrointestinal conditions and we may not be able to prescribe Mounjaro for you.

Patients with diabetes

Tirzepatide is not a substitute for insulin. Diabetic ketoacidosis has been reported in insulin-dependent patients whose insulin was stopped or reduced too quickly. We are unable to prescribe Mounjaro if you take insulin or a sulphonylurea (such as gliclazide), as these increase the risk of low blood sugar (hypoglycaemia). If your GP starts you on either while you are using Mounjaro, tell us straight away.

Diabetic retinopathy

Tirzepatide has not been studied in people with diabetic eye disease that needs active treatment, proliferative diabetic retinopathy or diabetic macular oedema. Rapid improvement in blood sugar control can temporarily worsen diabetic eye disease. If you have diabetic retinopathy, tell us during your consultation.

Interaction with other medicines

Tirzepatide slows stomach emptying and may affect how well other medicines taken by mouth are absorbed. This effect is greatest after your first dose and lessens over time. If you take a medicine that needs careful monitoring, such as warfarin or digoxin, tell us and your GP before starting, as you may need extra checks when you start treatment and after each dose increase.

Sodium

This medicine contains less than 1 mmol sodium (23 mg) per dose, i.e. it is essentially 'sodium-free'.

Mounjaro Frequently asked questions

Everything you need to know about Mounjaro

Mounjaro® (tirzepatide) is a once-weekly injection that activates the receptors for two natural gut hormones, GLP-1 and GIP. It helps reduce appetite and food intake, and it also lowers blood sugar. It is prescribed for adults living with obesity or who are overweight, alongside a reduced-calorie diet and more physical activity. Your prescriber tailors the dose to you.

Mounjaro is a prescription medicine for adults who want to lose weight and improve health problems linked to their weight. We may prescribe it for:

  • Adults with obesity: a body mass index (BMI) of 30 or more
  • Adults who are overweight: a BMI of 27 or more with at least one weight-related health condition, such as prediabetes or type 2 diabetes, high blood pressure, high cholesterol, obstructive sleep apnoea, heart disease, polycystic ovary syndrome (PCOS) or osteoarthritis

Mounjaro works alongside a reduced-calorie diet and increased physical activity to help you lose weight and keep it off. A prescriber reviews every consultation before treatment is approved.

Mounjaro is licensed for weight management in adults. As an online service, we only treat patients aged 18 to 75, because prescribing and monitoring at a distance needs extra precautions.
  • Inject once a week, on the same day each week, into your tummy, thigh or upper arm
  • You can inject at any time of day, with or without food
  • Change the injection site each week
  • Your dose starts low and increases gradually to reduce side effects
  • Use it alongside a reduced-calorie diet and increased physical activity for the best results
If you're new to Mounjaro, you need to start on 2.5 mg once a week for 4 weeks. This starting dose helps your body adjust and reduces side effects such as nausea. After that, the dose can go up in 2.5 mg steps, no more often than every 4 weeks, depending on how you respond and how well you tolerate it. The maximum dose is 15 mg once a week. Your prescriber reviews your progress at each check-in before changing your dose.

Yes. We can continue your Mounjaro treatment if you send evidence that you've used it within the last 6 weeks. We won't start you on a higher dose than your last confirmed one. If it's been longer, or you've missed several doses, your prescriber may restart you on a lower dose, such as 5 mg or the 2.5 mg starting dose. Acceptable evidence is an invoice or dispensing label from your previous provider that clearly shows:

  • Your full name
  • The date it was dispensed
  • The strength of your last Mounjaro order

Side effects such as nausea, diarrhoea and constipation are most common when you start or increase your dose, and they usually ease over time. To help:

  • Increase your dose gradually: only step up when your prescriber agrees. Staying on a dose for longer is fine if you're still losing weight.
  • Eat smaller meals: stop when you feel full, and go easy on fatty, fried or very sugary food.
  • Drink plenty of fluids: this helps prevent dehydration, especially if you have vomiting or diarrhoea.
  • Talk to us: our clinicians can suggest ways to manage side effects and adjust your plan if needed.

Losing weight with Mounjaro isn't just about appearance. Clinical trials have shown improvements in:

  • Blood pressure
  • Cholesterol and blood sugar levels
  • Mobility and physical function
  • Quality of life

Even a 5–10% weight loss can lower the risk of type 2 diabetes, heart disease and some cancers.

Wegovy® (semaglutide) activates the GLP-1 receptor. Mounjaro® (tirzepatide) activates both the GLP-1 and GIP receptors. Both are once-weekly injections used alongside diet and exercise. In the SURMOUNT-5 trial, which compared them directly over 72 weeks, people taking tirzepatide lost more weight on average: about 20% of body weight, compared with about 14% on semaglutide 2.4 mg. Your prescriber will help you choose based on your health, your goals and how you respond.
Yes. Many people switch because of cost, side effects or personal preference. There are no trials that test switching directly, so your prescriber will decide the right starting dose and timing for you, based on your current dose and how long it has been since your last injection.
It depends on your goals, your health, how you respond to treatment and cost. If you're progressing well on Mounjaro, staying on it is often the simplest choice. If cost is a concern, Wegovy can still be very effective, although average weight loss in trials was lower than with Mounjaro. Your prescriber will guide your plan, adjust your dose and monitor your progress.
Yes. We support you to safely reach a target BMI of around 23, then move you to a maintenance dose, depending on your clinical needs. You can stay on this dose for as long as it remains safe and appropriate, with ongoing monitoring. At this stage we also offer a clinician consultation to review your progress, goals and mindset, including whether and when you might want to reduce or stop treatment. We'll plan any changes with you, at a pace that suits you.

After you order Wegovy, your clinician will email you asking for:

  • BMI photos
  • Proof of your previous treatment, such as a dispensing label or invoice dated within the last 6 weeks

If your current BMI is under 25, you may also need evidence of your starting weight, such as a letter from your previous healthcare provider. This helps your clinician choose the right dose and plan a safe switch.

Yes, if you take the contraceptive pill. Mounjaro may make oral contraceptives less effective, especially when you start treatment or increase your dose. Switch to a non-oral method (such as a coil, implant or injection), or use a barrier method such as condoms as well as your pill:

  • for 4 weeks after starting Mounjaro
  • for 4 weeks after each dose increase

Mounjaro must not be used during pregnancy or breastfeeding. If you're planning a pregnancy, speak to us first: you'll need to stop Mounjaro at least 1 month before trying to conceive. If you become pregnant while taking it, stop and contact us straight away.

Possibly. Interim guidance from the British Menopause Society (BMS) covers women taking HRT alongside weight-loss injections such as Mounjaro. These medicines slow digestion, which may reduce how well tablets are absorbed.

  • Oestrogen: your GP or menopause specialist may suggest switching from tablets to a patch, gel or spray, which is absorbed through the skin.
  • Progesterone: if you take oral progesterone (such as Utrogestan), your GP may recommend doubling the dose for 4 weeks after you start Mounjaro and for 4 weeks after each dose increase, to keep protecting the womb lining. Alternatively, they may suggest a Mirena coil, which releases progestogen directly into the womb.

Important: never change your HRT dose yourself. Always speak to your GP or menopause specialist first.

Your pen is dispatched around day 28, so it arrives around the time your next dose is due. Delivery is free and your pen is sent in temperature-controlled packaging. Someone will need to be in to receive it, as we can't leave medicine in a safe place or with a neighbour.

Payments

Your saved card is charged automatically on day 28 of each cycle. You'll always be charged the Subscribe & Save price advertised on our website for your prescribed strength provided you’ve completed your check-in on time.
You won't pay for it. If a payment has already been taken for an order that isn't approved, we'll refund it in full.

Checking in is quick and simple, and we'll remind you along the way. We email you when your check-in opens on day 14, and send you reminders before day 24.

If you do miss one, you're covered. Your last check-in already set your preferred dose for your next pen, so we'll send it as normal at the Subscribe & Save price.

Your first check-in is the exception. As there are no earlier preferences to use, we can't send your next pen until you've completed it.

If you miss two check-ins in a row, we'll need you to check in before we can send your next pen.

Checking in by day 24 is part of the terms for the Subscribe & Save pricing. If you check in late and there are no preferences from your last check-in to cover you, we'll send your pen once you've checked in, charged at the one-off price.

Your next pen goes back to the Subscribe & Save price as long as you check in on time.

If you're not compliant with your check-ins or don't send information we ask for (such as BMI photos), your prescriber may end your subscription. If this happens within your first 6 months,an early exit fee* applies.
